Course Description
In-Office TMJ Surgical Treatment: Keys to Safe and Successful Outcomes …And Why We Do It!
John Nale, DDS, MD
This lecture will review surgical procedures used to treat internal derangements and degenerative joint disease. Historically these procedures have been done in a hospital setting, but over a decade of experience has shown the procedures to be safely performed in an “in-office” setting. A special emphasis in this discussion will be placed on techniques used to achieve desired outcomes.
Secondly, there will be a focus on safety, both perioperatively and post-operatively, to comfortably allow patients to go home as an outpatient. Lastly, we will review the many benefits of performing the surgeries outside of the hospital.
Non-Surgical Treatment of TMJ Disorders
Rana Almudamgha, DDS
This lecture, delivered by a distinguished orofacial pain and temporomandibular disorder (TMD) specialist, explores the multifaceted realm of nonsurgical management of TMD through a comprehensive lens. This presentation delves into a diverse array of conservative therapies, emphasizing the pivotal role of behavioral modification and lifestyle adjustments in the holistic treatment of these disorders. The discussion spans a spectrum of interventions, encompassing physical therapy, stress reduction techniques, targeted massage, prescription drugs, oral appliance therapy, patient education, transcutaneous electrical nerve stimulation (tens units), trigger point injections, dietary modifications, jaw exercises, psychotherapy, and thermal therapy.

The Role of Nutrition in Maxillofacial Healing
Teresa Biggerstaff, DDS, MD
Bone grafting and soft tissue grafting techniques have become more and more sophisticated over time due to the innovative development and use of biomaterials. These regenerative procedures allow for dental rehabilitation utilizing dental implants. Careful patient selection, surgical technique, and selection of appropriate biomaterials and sound functional design are the foundational principles of predictable outcomes. But sometimes, even when all of these guiding principles are followed, we still see failure of the regenerative procedure or failure of the dental implant. Increasing evidence suggests that a lack of certain micronutrients may contribute to these failures. We will review the effects of micronutrients on bone health, bone grafting, periodontal health, soft tissue healing, implant osseointegration and immune regulation and discuss strategies to optimize serum levels of micronutrients to prepare for regenerative procedures. As our society continues to rely more and more on processed foods and fast foods that are less nutrient-dense, we are seeing an increase in micronutrient deficiencies, especially in our patients who are more advanced in age. These patients tend to be the ones most commonly seeking dental rehabilitation with dental implants.
Anesthesia Update – Incorporating DEXmedetomidine into the Office-based Anesthesia Model Provided by the Oral and Maxillofacial Surgeon

As implant dentistry continues its rapid evolution, clinicians regularly encounter new treatment trends and concepts. Success is dependent on the ability to decipher between the myths and realities through careful evaluation of both treatment modalities and new technologies. This presentation will address a range of treatment concepts and implant technologies through a combination of clinical research review and real-world experience.

Rana Almudamgha, DDS
Rana Almudamgha, BDS, FAAOP is an Assistant Professor Clinical in the division of the Comprehensive Dentistry Department at LSU School of Dentistry. She serves as a Program Director of the developing Advanced Education Program in Orofacial Pain and a Director of the Temporomandibular Disorders and Orofacial Pain Clinic. At LSU School of Dentistry, she provides Orofacial Pain patient counseling and teaches pre-doctoral and post- doctoral dental students in the field of Orofacial Pain. Before assuming the position, Dr. Almudamgha served as Gratis faculty at the LSU Health Sciences Center New Orleans. Dr. Almudamgha holds a Bachelor of Dental Surgery degree from the University of Baghdad, College of Dentistry. Leveraging her lifelong passion for dentistry and holistic patient care, she completed her Advanced Education Program in Orofacial Pain at the Eastman Institute for Oral Health at the University of Rochester in 2019. Before arriving in the United States, Dr. Almudamgha completed her general practice residency in Iraq and then worked as a general practitioner in private practice in Amman, Jordan. Dr. Almudamgha is board-certified by the American Board of Orofacial Pain, a fellow of the American Academy of Orofacial Pain, and a member of the American Dental Association, American Dental Education Association, and the Louisiana Dental Association.
Teresa Biggerstaff, DDS, MD
A native of Washington State began her training at the University of California, San Francisco School of Dentistry. She completed her Oral and Maxillofacial Surgery training and medical school at LSU in New Orleans. Training at LSU allowed Dr. Biggerstaff to evaluate and treat the entire spectrum of oral and maxillofacial problems. Following completion of residency, she moved to North Carolina for a fellowship in Facial Cosmetic Surgery before joining a private practice in Winston-Salem and has been in private practice in North Carolina ever since. She enjoys private practice and hosting and teaching residents in the Osteoscience Observership program. She maintains a very busy oral surgery and facial surgery practice in two locations and owns and directs a medical spa as well. Dr. Biggerstaff is one of two speakers for the International Association of Oral and Maxillofacial Surgeons annual Webinar. She speaks on Nutrition and Healing. This is broadcast to all IAOMS members with an audience of over 300 registered people. She has become a recognized authority on this topic and the only OMS who speaks on this topic. Dr. Biggerstaff serves on the executive committee of the NC Society of Oral and Maxillofacial Surgeons and serves on multiple committees for the Southeastern Society of Oral and Maxillofacial Surgeons
David Bulot, DDS, MD
A native of Louisiana. He attended Nicholls State University for his undergraduate studies. He received both his Doctorate of Dental Surgery and his Doctorate of Medicine degrees from Louisiana State University Health Sciences Center in New Orleans, Louisiana. He earned his Board Certification in Oral and Maxillofacial Surgery in 2013. He has a special interest in teaching the LSU dental students a comprehensive approach to the dental patient and regularly teaches at LSU Dental School. He also sponsors and leads annual continuing education courses in Baton Rouge with a special interest in comprehensive implant dentistry and well as the “Oral Surgeon Advantage” for wisdom tooth extraction.
John Wesley (Wes) Emison MD, DDS
A Clinical Assistant Professor at Louisiana State University Department of Oral and Maxillofacial Surgery, Dr. Emison completed his undergraduate education at California Western University in San Diego, California. He received his D.D.S. from the UCLA School of Dentistry. Dr. Emison then completed his residency in Oral and Maxillofacial Surgery (OMFS) at Charity Hospital of New Orleans with the renowned Louisiana State University (LSU) Department of Oral and Maxillofacial Surgery training program. He subsequently attended medical school at the LSU School of Medicine, becoming the first dual-degree OMFS in Louisiana. Following medical school, Dr. Emison completed his general surgery internship at Ochsner Hospital of New Orleans. Dr. Emison was in the private practice of OMFS in Los Gatos, California for 34 years where he was the Medical Director of the QUAD A accredited ambulatory surgical facility known as Los Gatos Oral and Maxillofacial Surgery. Additionally, Dr. Emison was Chair of the Division of Oral and Maxillofacial Surgery in the Department of Surgery at Good Samaritan Hospital in San Jose, California for 25 years. Dr. Emison is now a Clinical Assistant Professor and attending surgeon at University Medical Center New Orleans. He is a founding member and the current President of the Jack Kent Oral and Maxillofacial Surgery Foundation.
Jeffrey James, MD, DDS, MBA, FACS, FAACS
Dr. James is the Peltier Endowed Chairman and Head of the Oral and Maxillofacial Surgery Department at Louisiana State University (LSU) Health Science Center in New Orleans. He is a full Professor of Tenure at LSU and has over 10 years of private practice and academic surgery experience. Dr. James continued his education at the University of Oklahoma where he completed a fellowship and obtained a Certificate in Cleft and Craniofacial Surgery. He also trained at the Arnold Palmer Children’s Hospital in Orlando to expand his expertise to include cranial vault and fronto-orbital reconstruction for craniofacial dysmorphologies. Dr. James then trained at Cosmetic Surgery Affiliates furthering his competency as a facial cosmetic surgeon. Most recently, Dr. James graduated top of his class at the Hull College of Business and earned a Master’s in Business Administration (MBA). Dr. James is dual board certified. He is Board Certified in Oral and Maxillofacial Surgery as well as Facial Cosmetic Surgery. He also currently serves as a Board Examiner for the American Board of Facial Cosmetic Surgery. Dr. James is a Fellow of
The American Association of Oral and Maxillofacial Surgeons, The American College of Surgeons, and the American Academy of Cosmetic Surgeons. He has maintained membership in the American Cleft Palate–Craniofacial Association, American Academy of Cosmetic Surgeons, American College of Surgeons, American Association of Oral and Maxillofacial Surgeons, American Medical Association, and the American Dental Association. Dr. James is proud to have served his Country in the United States Air Force Reserves as a staff surgeon for the Air Force and Army Maxillofacial Surgery Training Program for 10 years.
Thomas Kiebach DDS MSD
A Prosthodontist born and raised in Baton Rouge. He attended Louisiana State University for his undergraduate studies and received a bachelor’s degree in biological sciences with minor concentrations in psychology and chemistry. He received his Doctor of Dental Surgery (DDS) degree from LSUSD in New Orleans. After dental school, he worked in private practice as a general dentist for several years before returning to LSUSD, where he completed a 3-year residency in the department of prosthodontics. He received extensive training in full mouth rehabilitation, implant prosthetics, occlusion, TMD, dentofacial esthetics, advanced treatment planning, and advanced digital dentistry. As part of his residency, he received a Master’s of dental science degree. This extensive training allows Dr. Kiebach to provide a variety of comprehensive restorative treatment options, utilizing the most advanced technology available. Dr. Kiebach has a special interest in the use of digital technology to provide enhanced diagnostic and treatment planning capabilities, create synergy between the clinical team and the laboratory, and ultimately provide superior patient experience. His passion is in simplifying the process of creating a virtual patient (digital clone) through the fusion of multiple streams of data coming from various digital technologies such as computed tomography (3D X-rays), digital photography, *3D facial scanners, intraoral scanners, 4D jaw motion tracking, digital CAD design software,
and 3D printers. Armed with the superior diagnostic data achieved with the virtual patient creation, our interdisciplinary team of specialists can create customized surgical and prosthetic treatments in a fraction of the time.
John Nale, DDS, MD
John Nale has been a surgeon at Carolinas Center for Oral and Facial Surgery in Charlotte, NC for over 13 years. He practices full-scope oral surgery with a concentration in TMJ Surgery. He has authored chapters on the management of TMD patients and the management of orthognathic patients with TMD. He was also the instructor for TMJ surgery at both the Denver and LSU Oral Surgery review courses.
Steven Nelson, DDS, MD
An oral and maxillofacial surgeon who was raised in Baton Rouge. He attended Louisiana State University for his undergraduate studies and received a bachelor’s degree in biological sciences. He received his Doctor of Dental Surgery (DDS) degree from Louisiana State University School of Dentistry in New Orleans, Louisiana. In addition to graduating at the top of his dental school class, he was bestowed the Horace Wells Excellence in Dental Anesthesia Award, and the Excellence in Implant Dentistry Award. He was also inducted into the C. Edmund Kells and OKU Dental Honor Societies. Dr. Nelson has a special interest in orthognathic surgery, bone grafting, soft tissue grafting, and dental implant surgery. Along with his well-trained office staff, he can provide his patients with the most updated and accurate methods of care using in-office CT scans and the latest digital equipment.
